To Louisville office: We are located across from Baptist East Hospital in the building with the GREEN stripe (4001 Baptist East Office Park). Address is 4001 Kresge Way, suite 310. Kresge Way is just off of Breckenridge Lane. Coming from I-64, exit at Breckenridge Lane, head west towards the city (the Dupont area). At the 3rd light, look for Kresge Way on the right and turn here. Baptist Hospital will be on the right and our building will be on the left. Take elevator to 3rd floor. To New Albany office:We are located directly across the street from Floyd Memorial Hospital in the Professional Arts building (1919 State St) on the first floor. Coming from Louisville, get on I-64, follow this across the river (double-deck bridge...do not take I-65 bridge). Immediately after crossing the river, take the first exit, then merge into left lane. At the second light, turn left on State St. Proceed about 1.5 miles, and you will see the hospital on right. Turn left into the small side street once you see the 3-story Professional Arts Building (Our office is right on ground level corner of main building). Proceed to parking booth (you will be asked for $1 parking fee). Proceed to first floor at about center of building complex. ( suite #144). If already in Indiana, from I-265, take the State St exit, head south about 2 miles, and look for hospital on left and our building on right. Phone 812 -945-3223.
